VISUAL/MEDIA ARTS PROJECT application for organizations and collectives
Applicants to the project grant program must download 3 documents for a complete application package.
- The Guidelines and application form are one document, formatted as a Word template1 with some pages designed as forms to be filled out on your computer. However, you will still submit much of the required information using attached pages.
- The Financial/Statistical form is available in both PDF and Excel format. You can download the Excel file to your desktop and complete the form on your computer, or you can print the PDF version and fill out the form by hand. Select one only.
- The Declaration of Non-Discrimination is in PDF format so you can print and sign it.
1 Word template files have a .dot file name extension, rather than the usual .doc extension.
